Your Cardiologist Is Staring at the Wrong Number — The Inflammation Tests That Actually Predict Heart Attacks
Let's set the scene. You go in for your annual physical, your doctor orders a lipid panel, and a few days later a nurse calls to tell you your cholesterol looks "totally fine." You hang up feeling relieved. Maybe you even celebrate with a cheeseburger.
But here's the thing nobody mentioned: a significant portion of people who suffer heart attacks have perfectly normal LDL cholesterol. Lipoprotein(a), or Lp(a), is a cholesterol particle with a sticky protein attached to it. It's highly heritable — meaning if your parents had elevated levels, there's a solid chance you do too — and it's independently associated with a dramatically increased risk of heart attack and stroke. Some researchers consider it one of the most dangerous cardiovascular risk factors in existence.
Here's the kicker: Lp(a) levels don't respond meaningfully to diet, exercise, or standard statin therapy. You can do everything "right" and still have dangerously elevated Lp(a) because it's largely written into your DNA. hs-CRP is cheap. It's widely available. And yet it's still not part of a standard lipid panel. You have to specifically ask for it, and even then some doctors will push back, telling you it's "not necessary." You're allowed to disagree.
Homocysteine: The Amino Acid Your Heart Hates
Homocysteine is an amino acid that accumulates in the blood when certain B vitamins — particularly B6, B12, and folate — are deficient or not being properly metabolized. Elevated homocysteine is directly toxic to the endothelium, the thin layer of cells lining your blood vessels. Think of it as sandpaper scraping the inside of your arteries, creating the micro-injuries that inflammation then rushes in to "repair" — often in ways that accelerate plaque formation.
The connection between high homocysteine and cardiovascular disease has been documented in research since the 1990s. And yet homocysteine testing still isn't routine. It's another marker that requires a specific request, and one that's particularly relevant if you're on certain medications (including metformin and some acid blockers) that can deplete B vitamins over time.

Part of it is inertia. Medical guidelines move slowly, and the infrastructure of routine preventive care was built around a cholesterol-centric model that's been in place for decades. Changing what gets ordered on a standard lab requisition involves updating guidelines, retraining physicians, adjusting insurance billing codes, and a dozen other bureaucratic hurdles that have nothing to do with patient outcomes.

And part of it, frankly, is that the pharmaceutical industry has invested heavily in the LDL story. Statins are among the most prescribed drugs in American history. Shifting the conversation toward inflammation opens up a more complicated picture — one that doesn't necessarily have a simple pill-based solution.

First, stop waiting for your doctor to bring this up. They may never. That's not necessarily negligence — it's often just the reality of a 15-minute appointment driven by standard-of-care guidelines that haven't fully caught up with the research.
Before your next physical, write down these three tests and ask for them by name:
- Lp(a) — especially if you have any family history of early cardiovascular events
- hs-CRP — a baseline inflammation check that should arguably be routine for every adult over 40
- Homocysteine — particularly relevant if you're on long-term medications, follow a plant-based diet, or have known B12 absorption issues
If your doctor resists, ask them to document that refusal in your chart. That request alone often changes the conversation.
