Somewhere in a doctor's office right now, a woman in her late forties is being handed a standard hormone replacement therapy prescription. Maybe it's a patch. Maybe it's a pill. The dose is almost certainly the same dose her neighbor got, the same dose the woman three states over got, and — here's the part nobody really talks about — roughly the same dose that's been handed out for decades, calibrated from studies that were never actually designed to capture the full complexity of what happens inside an individual woman's body.

That's not a conspiracy theory. That's just the history of how HRT developed, and it matters enormously if you're one of the millions of American women who has either felt worse on hormone therapy than you expected, or felt no meaningful improvement at all.

The Study That Shaped Everything — And Its Limits

Most conversations about HRT eventually circle back to the Women's Health Initiative, the massive federally funded study that was halted early in 2002 after researchers flagged elevated risks for breast cancer, stroke, and blood clots in participants taking a specific combination of synthetic estrogen and progestin. The fallout was immediate and dramatic. Prescriptions dropped. Women stopped asking. Doctors stopped offering.

What got lost in the panic was a more nuanced read of the data. The WHI studied a very particular population — women who were, on average, 63 years old at enrollment, more than a decade past the typical onset of menopause. The hormones used were synthetic conjugated equine estrogen and medroxyprogesterone acetate, not the bioidentical hormones that many practitioners now favor. And the dosing? Uniform. Everyone got the same amount regardless of body weight, symptom severity, liver function, or genetic profile.

That's a lot of variables being ignored in research that went on to define clinical practice for an entire generation.

Your Liver Is Running a Different Program Than Hers

Here's something conventional HRT prescribing rarely accounts for: the way your body processes estrogen is not the same as the way another woman processes it. Estrogen metabolism runs through the liver, and how efficiently your liver handles that process depends on genetics, gut microbiome composition, stress load, thyroid function, and a handful of other factors that a standard intake form isn't going to capture.

Some women are fast metabolizers — they clear hormones quickly, which means a standard dose may barely register before it's gone. Others are slow metabolizers, and the same dose accumulates in ways that can tip the balance toward unwanted effects. Neither of these women is broken. They just have different biological operating systems.

Oral estrogen, the most commonly prescribed form, passes through the liver before it ever reaches circulation. That first-pass metabolism changes the hormone significantly and creates byproducts that don't exist when estrogen enters the body transdermally — through the skin. Transdermal delivery bypasses the liver entirely, which is one reason many practitioners who specialize in women's hormonal health have shifted away from oral forms. But that shift hasn't made it into the standard playbook yet.

Saliva Testing and What It's Actually Measuring

Conventional hormone testing almost always uses serum — a blood draw that measures total hormone levels circulating in the bloodstream. That number tells you something, but it doesn't tell you everything. Specifically, it doesn't tell you how much of that hormone is actually bioavailable — able to enter your cells and do its job.

Saliva testing measures what's called free hormone — the active, unbound fraction that's actually in play at the tissue level. Critics of saliva testing argue it's less standardized and harder to interpret, and those are fair points worth weighing. But proponents — and there are a growing number of clinicians in this camp — argue that for women on topical hormone preparations especially, saliva testing gives a more accurate picture of what's biologically active.

The gap between total circulating hormone and free bioavailable hormone can be significant. A woman whose blood work looks fine might be functionally running on empty at the cellular level. That's not a hypothetical edge case — it's something practitioners who use more comprehensive testing panels encounter regularly.

Compounded Bioidentical Hormones: Promising or Overhyped?

Compounded bioidentical hormone therapy — custom-formulated hormones made by a compounding pharmacy to match a specific patient's needs — has become something of a cultural flashpoint. Critics, including some major medical organizations, argue that compounded formulations lack the standardized safety and efficacy data that FDA-approved products carry. That's a legitimate concern that deserves honest engagement.

But the argument on the other side is equally worth sitting with: FDA-approved HRT products come in a handful of fixed doses. If your biology requires something in between those options — a lower starting dose, a different ratio, a delivery method that doesn't exist in a commercial product — compounded formulations are sometimes the only way to get there.

The real question isn't whether compounded hormones are categorically better or worse. It's whether the current system of standardized dosing is actually serving the full range of women who need support. And the evidence that it isn't is sitting in every forum thread, every Facebook group, every conversation between women comparing notes on why their symptoms didn't budge even after starting treatment.

The Symptom Picture Conventional Dosing Keeps Missing

Menopause isn't a single experience. For some women, it's primarily about hot flashes and disrupted sleep. For others, it's cognitive fog, joint pain, mood instability, or heart palpitations. For many, it's all of the above in a rotating combination that shifts from month to month.

Standard HRT protocols were largely calibrated to address vasomotor symptoms — the hot flashes and night sweats that show up in clinical trials because they're measurable and reportable. The subtler presentations — the word-finding difficulties, the anxiety that came out of nowhere, the fatigue that no amount of sleep touches — are harder to quantify, which means they're often harder to treat within a system that doesn't have a standardized response to them.

That leaves a lot of women either being told their symptoms aren't hormone-related, or being handed the same standard prescription and told to give it three months before reassessing. Three months is a long time to feel terrible when the underlying issue might be that the dose is wrong for your specific biology.

What Asking More Questions Looks Like in Practice

If you're navigating HRT and feeling like something isn't quite right, the renegade move here is actually pretty straightforward: ask more questions. Ask about testing options beyond standard serum panels. Ask whether transdermal delivery might be a better fit than oral. Ask what the process looks like for adjusting your dose based on symptoms rather than just lab numbers.

Find a practitioner — whether that's a functional medicine doctor, a menopause specialist, or a gynecologist who's done additional training in hormonal health — who treats your lab results as one input among many rather than the whole story.

The conventional model isn't entirely wrong. Hormone therapy, when it fits, can be genuinely transformative. The problem isn't the therapy itself. It's the assumption that a protocol built on population averages is going to land correctly for every individual woman who walks through the door.
